Carson City Health Department's food-service inspections and the date of inspection (all scores are on a 100-point scale, with points deducted depending on the severity of violations):

• BPO Elks Lodge No. 2177, 515 N. Nevada Street, food facility fee exempt, bar, scored 100 points.

• Dotty's No. 10, 2280 E. Highway 50, bar/lounge, scored 100 points.

Eagle Valley Market, 933 Woodside Drive, No. 105, convenience Store, scored 98 points. The ice machine drip plate had a build-up of debris.

• Panaderia Azteca Bakery, 3189 E. Highway 50, retail grocery, scored 100 points.

• Panaderia Azteca Bakery, 3189 E. Highway 50, bakery, scored 95 points. Mixed doughs were found on a tray on a rack without any cover to protect from contamination. The 3-compartment sink has a large space between it and the wall. Non-food contact surfaces that are subject to splash or food debris from the three-sink are difficult to clean. A supply of paper towels were not present at the hand sink.

• Panaderia Azteca Bakery, 3189 E. Highway 50, take out, scored 97 points. No container of sanitizer was available to use with wiping cloths. Food contact surfaces of the cutting board on the prep table and nearby utensils were not clean and contained food debris.

• Super Mercado "El Torito", 604 E. Winnie Lane No 308, bakery, scored 94 points. Improper procedures are used when washing items/utensils in the three compartment sink. The third sink was not used with sanitizer. Some food (salsa ingredients, chicarrones) were stored in containers without covers.

• Super Mercado "El Torito", 604 E. Winnie Lane No 308, meat department, scored 93 points. Pig meat was found stored in a 5-gallon detergent bucket. Food contact surfaces of the meat slicer was not clean.

• Super Mercado "El Torito", 604 E. Winnie Lane No 308, retail grocery, scored 99 points. In-use dispensing utensils for the bulk food are not stored properly. The scoop handle was in the food.

• Arco AM/PM No. 82392, convenience store, scored 100 points.

• Dutch Brothers Coffee, 1449 S. Carson Street, take out, coffee house/drive through, scored 98 points. The ice machine drip plate was not clean and the ice scoop handle was in the ice.

• Locals Barbecue Company LLC, 3849 S. Carson Street, restaurant, scored 100 points.

• MJ's Sports Bar, 3679 S. Carson Street, bar/lounge, scored 99 points. Cloths used on food-contact surfaces did not contain sanitizer residual.

Katie's Bar, 3481 E. Highway 50, bar/lounge, scored 98 points. Found speed gun and holder with mold growth present. The holder is not draining properly through the drain hose. (Repeat violation) Found extensive mold growth inside the entire ice machine. (Repeat violation)

• Taqueria La Salsa, 1946 E. Highway 50, restaurant, scored 92 points. Found shredded cooked chicken and pork in the top make-up portion of the reach-in at 52F and 54.5F, respectively. This unit is holding an ambient air temperature of 47.5F. (Note: The unit cooled down to 40F within 30 minutes during this inspection.) Found numerous containers of food in reach-in units with no labels noting current contents or dates prepared. Found mold build-up on deflector shield and in top half of the main ice machine. Tray holding ice scoop at main ice machine needs cleaning.

• Madeleine's Cookies, 3680 Research Way No. 5, retail food producer, scored 100 points.

• Novedades Gutierrez, 1501 E. Fifth Street No. 102, convenience store, scored 100 points.

• National's Cafe, 1701 N. Carson Street, restaurant, scored 94 points. Found cooked, minced scallops at 48.4F on the make-up line. Found no measurable sanitizer solution in the sanitizer buckets at the sushi prep/service area. Sushi knives are just being wiped with damp cloths between orders.

• Basil The, 311 N. Carson Street, restaurant, scored 89 points. Found three bags of packaged raw chicken in a tub near the walk-in at a temperature of 67F. According to staff, this product was taken from the walk-in approximately 1.5 hours prior. Found the three-compartment sink improperly set up during active dishwashing. There was no sanitizer in the water in the third compartment. Also found the chlorine sanitizer feeding improperly to the dishwsher after numerous wash cycles and priming. So there was no proper sanitation of dishes provided/available during active food preparation in the kitchen. Certified Food Manager credential is expired.

• Murphy Bernardini, 1545 E. Fifth Street, food facility, scored 100 points.

• Fish Food Bank, 138 E. Long Street, food facility fee exempt, scored 100 points.

• Lady Tamales, The, 933 E. Woodside Drive No. 102, restaurant, scored 98 points. No Certified Food Protection Manager is on duty.

• Fish Family Dining, 138 E. Long Street, food facility fee exempt, scored 98 points. Found ice scoop at main ice machine stored in a dirty bowl. Found calcium carbonate build-up in the main ice machine.

• Bully's, 3530 N. Carson Street, bar/lounge, scored 100 points.

• Bully's, 3530 N. Carson Street, restaurant, scored 96 points. Found unlabeled containers of foods in the walk-in. Observed small cups being used to dispense dry foods (sugar, flour). (Repeat violation) Observed substantial mold build-up in the upper half of both ice machines and on a deflector shield in the lower half of one of the machines.

• Mercado San Juan, 137 Clearview Drive, meat department, scored 100 points.

• Dotty's No. 12, 3771 S. Carson Street, bar/lounge, scored 100 points.

• CVS/Pharmacy No. 8779, 220 Fairview Drive, convenience store, scored 100 points.

• Kabayan Multi-Mart, 321 Fairview Drive, retail grocery, scored 100 points.

• Sub Factory, The, 2589 N. Carson Street No. B, restaurant, scored 95 points. Found thermometers missing from most of the refrigerated units. Found no sanitizer buckets set up to store wiping cloths in between use. Found abundant mold inside the main ice machine. (Repeat violation) Observed standing water (with rust and mold) in the bottom of the reach-in bulk meat cooler. Observed old meat residue in several hard-to-reach areas of the meat slicer. Found single service items (cups, napkins, etc.) stored on open shelving in the ladies' room.

• McDonalds No. 2282, 2001 N. Carson Street, restaurant, scored 92 points. The three-compartment sink is improperly set up during active dishwashing. Found mold growing inside the tubing in the upper half of the main ice machine. A very damaged ice scoop is being used at the main ice machine. Found mold in the bottom crevice of the ice scoop holder. Found no residual sanitizer solution (chlorine) in the buckets holding wiping cloths. Observed staff on the sandwich make-up line using wiping cloths to wipe food from hands, then wipe the prep surface, then store cloths on the prep surface instead of returning the cloth to the sanitizer bucket. One contributing factor to this behavior is the distant location of the only kitchen handsink. (Repeat violation) Black drain pipes located in the ceiling of the basement are leaking into the buckets.

• Capital Christian School, 1600 Snyder Avenue, school cafeteria fee exempt, scored 98 points. An accurate metal stem thermometer is not provided to check food temperatures. In-use dispensing utensils are not stored properly.

• Sizzler Restaurant, 3033 N. Carson Street, restaurant, scored 99 points. Found grease build-up between grills on cook line. Several small areas need re-caulking at the wall sink contact in the dishwashing area. Found gap in the coving in the corner under the coffee station.

• McDonalds No. 4, 3344 N. Carson Street, restaurant, scored 96 points. Found ice scoop at the main ice machine improperly stored. The cover is missing from the top half of the ice machine located at the back kitchen door. There is a build-up of food debris at the drain basket inside the dishwasher. Mop sink faucet is leaking. The bottom of the mop sink is damaged by three holes and is uncleanable and not draining properly.

• Donuts to Go, 1413 N. Carson Street, scored 97 points. Found unlabeled containers of dry foods (sugar, flour, etc.). Found soap and bleach in the only sanitizer bucket holding wiping cloths. The floor sink behind the dairy reach-in is dirty.

• Jenny Craig Weight Loss Center, 444 E. William Street, convenience store, scored 100 points.

• Carson City Municipal Golf Course, 3999 Centennial Park Drive, bar, scored 100 points.

• 4 Pints Tavern & Grill, 3198 N. Deer Run Road, bar/lounge, scored 98 points. A supply of paper towels were not present at the bar hand wash sink.

• Seven Eleven No. 2236-32303A, 3838 E. Highway 50, convenience store, scored 97 points. Cleaning and maintenance tools are not properly stored. The mop bucket was found with dirty water in it. No certified food handler was present at the time of inspection.
