REQUEST FOR PROPOSALS 

AUDIOLOGY, ENDODONTICS, AND PEDIATRIC DENTISTRY 

Fallon Paiute-Shoshone Tribe 

Date of Issuance: June 10, 2024 

The Fallon Paiute-Shoshone Tribe, on behalf of the Fallon Tribal Health Center, is requesting proposals from qualified individuals/firms to provide on-site healthcare services for the following: 

AUDIOLOGY 

ENDODONTICS 

PEDIATRIC DENTISTRY 

Please contact Jon Pishion, Health Director, by email at fthcdirector@fpst.org, to receive a complete RFP package. Responses are due by Friday, July 26, 2024, by 4:00 pm PDT. Proposals not received after that time and date shall be rejected. 

In accordance with 25 CFR and Tribal TERO Ordinance, Indian Preference will be afforded to qualified Native American applicants/firms. The Fallon Paiute-Shoshone Tribe reserves the right to accept and or refuse any and/or all responses.
