Hands being dipped in sanitizer buckets in lieu of washing hands properly in a hand wash sink. Hot water turned off at hand wash sink. Hand wash sink being used as a dump sink for discarded vegetables. Cloths used on food-contact surfaces are not clean, do not contain sanitizer residual, or are not used properly. Wiping cloths laying all over counter tops, food prep areas, in reach in refrigerator next to eggs, and wrapped around dish wash sprayer hose.
