Carson City Health Department's food-service inspections for the week of June 29 - July 13 (all scores are on a 100-point scale, with points deducted depending on the severity of violations):

Playa Azul, 415 E. William St., restaurant, scored 94 points. Salsas and vegetables at the self-serve bar unrefrigerated. Serving spoon laying in/on the container of Spanish rice in the walkin. Sauces produced on site are labeled as to contents, but still need the date produced also on the container label. Foods stored in cracked containers, containers with partial lids and lids that did not adequately cover the product. Foods stored in non-food grade plastic bags. Three-compartment sink is not properly set up during active dish washing operation. Cloths used on food-contact surfaces are not clean, do not contain sanitizer residual, or are not used properly. There are not chlorine est strips to monitor the concentration of the sanitizer in the 3-C sink or the cloth buckets. Floor tiles are missing in the dish washing area, at the back door, and in the ladies restroom. Hand sink in the ladies' restroom is detached from the wall. Weather-stripping does not form an adequate seal at the back door.

Subway #28790, 2073, U.S. 50 East, scored 93 points. Found meats and cheeses on the sandwich makeup line at 50-52 degrees fahrenheit. Equipment and utensils are not handled or stored in a manner to prevent contamination. Tubs holding clean food storage container lids contain dirty water residue. Floor sink under the soda dispenser where clean customer cups are stored has thick slime buildup.

Dairy Queen of Carson City, 3198 U.S. 50 East, scored 94 points. Readily perishable cold food is held above 46 degrees fahrenheit. The meat cold-cuts are at 50-51 degrees fahrenheit (internal temperature) inside the food prep makeup unit. The ambient internal temperature of this unit is at 54.7 degrees fahrenheit. Cloths used on food-contact surfaces are not clean, do not contain sanitizer residual or are not used properly.

Taqueria La Salsa, 1946 U.S. 50 East, scored 93 points. Shredded cooked meats in the top of the makeup unit at internal temperatures of 51-60 degrees fahrenheit. The ambient internal temperature of this unit is at 43 degrees fahrenheit (after the lunch rush). Seafoods in the same unit were at 45 degrees fahrenheit (surface temperature.) At least one employee of this establishment must be a certified food handler.

Molly's Catering & Gourmet, 220 W. John St., scored 99 points. Facilities for manual dishwashing are not properly designed, constructed, maintained, installed, located or operated. There is wash water in one basin and clean dishes draining in another of the basins of the 3-C sink.

Butler Meats, 1909 N. Carson St., scored 100 points.

National's Cafe, 1701 N. Carson St., scored 92 points. Readily perishable cold food is held above 46 degrees fahrenheit. The prepared egg rolls on top of the makeup unit in the sushi area are an internal temperature of 55 degrees fahrenheit, and are not being maintained at 40 degrees fahrenheit or below. Observed two areas in the kitchen where meats were being thawed improperly. Cloths used on food-contact surfaces are not clean, do not contain sanitizer residual, or are not used properly.

Jack In the Box #7304, 3665 S. Carson St., scored 100 points.

Old Globe Saloon, 407 N. Curry St., scored 94 points. Chlorine residual of less than 50 ppm was detected on dishware after mechanical final rinse. Food contact surfaces speed gun and speed gun holster contained debris.

Juan's Mexican Grill #2, 3680 Goni Road, scored 100 points.

Governor's Field Food Facility, 500 Evalyn Dr., scored 100 points.

Sonic Drivein, 1856 E. College Parkway, scored 100 points.

Edner of Nevada Retail Food Producer, 2801 Conestoga Dr., scored 99 points. Floorsare not constructed of smooth durable material or are not maintained in good repair.

Silver Dollar Casino Sky Bar, 1897 N. Edmonds Dr., scored 100 points.

Silver Dollar Casino Blue Bar, 1897 N. Edmonds Dr., scored 97 points. Food contact surfaces of equipment or utensils are not clean. The ice machine is dirty inside. The floor sink under the soda dispenser needs cleaning. Coving tiles are loose around the above/same floor sink. The perimeter of the floor in this bar needs cleaning.

Silver Dollar Casino Red Bar, 1897 N. Edmonds Dr., scored 100 points.

Woody's Deli & Market, 4385 S. Carson St., scored 100 points.

Joanie's Creekside Cafe & Catering Restaurant, 1795 E. College Parkway, scored 96 points. Milk and whipped cream were found in the small refrigerator at the espresso bar between 41-45 degrees fahrenheit and not at 40 degrees fahrenheit or less. A supply of soap and paper towels was not present at the handwash sink at the espresso bar.

Joanie's Creekside Cafe & Catering Caterer, 1795 E. College Parkway, scored 100 points.

Smith's Food & Drug Meat Department, 599 E. William St., scored 98 points. Floor tiles and coving by door in cutting room not maintained in good repair. Coving is broken off the wall. Floor tiles and coving have large space between them in the seafood department and are not maintained in good repair.
