In about three weeks, the fields and courts at Churchill County High School will be buzzing.

On Aug. 14, the first official day of fall practice for the 2014 season begins.

With that, athletes must submit their athletic registration packets before being allowed to participate.

Student-athletes must file the packets with the CCHS office from Aug. 11-14 and enclose $80 for an athletic registration fee. The registration fee includes $50 for a participation fee and $15 each for an impact concussion fee and student body card.

The office will be open from 8 a.m.-noon and 1-4 p.m. each day. A $20 late fee will be added to packets received after the Aug. 14 deadline. Packets must be delivered in person during the established dates.

The forms consist of medical history, sport registry, residency statement, offseason conditioning agreement, Nevada Interscholastic Activities Association health questionnaire, CCHS drug, alcohol and tobacco, athletic code, insurance, hazing policy and athletic trainer consent.

Incoming freshmen, juniors and new students must complete the NIAA B (pre-participation history) and D (physical exam) forms. Sophomores and seniors are exempt from those forms.

To practice, a student-athlete — upon submitting the packet to CCHS — must receive an athletic participation clearance from Athletic Director Brad Daum and the CCHS athletic department.

Packets will not be accepted by coaches and if an athlete turns one in on Aug. 14, they may not be able to practice that day.

Physicals must be current and exams from a previous season, the middle school or Fallon youth league will not be accepted. Athletes must have insurance coverage prior to practicing and trying out for a sport.

For information or questions regarding the athletic registration packet, call CCHS at 775-423-2181, ext. 110.

Packets can be downloaded on the CCHS website.
